چکیده
Progression of Diabetic Retinopathy in theHypertension InterventionNurse Telemedicine Study Interventions to improveglycemic control reduce theprogression of diabetic retinopathy.1 Additionally, pharmacologic reductionof bloodpressure (BP) inpatientswithdiabetesmellitus and poorly controlled hypertension reduces the risk of worsening diabetic retinopathy.2 Despite pharmacologic improvements, hypertension remains poorly controlled in approximately half of Americans with it.3 The Hypertension Intervention Nurse Telemedicine Study (HINTS) investigated a telemedicine-mediatedmedicationmanagement and behavioral intervention for hypertension. Participants inHINTS (includingnondiabetic individuals)withpoorBP controlwho received combined medication and behavioral management demonstratedmeandecreases in systolic BP of 15mmHgand 8 mm Hg at 12 and 18 months, respectively.4 The purpose of the current analysis was to determine whether the interventions influenced theprogressionofdiabetic retinopathy inparticipants with diabetes.

BACKGROUND Diabetic retinopathy (DR) is the leading cause of acquired blindness in U.S. adults. Early detection prevents progression. Screening rates are only 10% in medically underserved populations. METHODS A statewide telemedicine-based program within primary care centers was implemented to improve DR screening, detection and referrals for underserved patients. STUDY DESIGN Retrospective...

moderate reduction in systolic BP achieved in HINTS via the nurse-administered telemedicine program4 (8 mm Hg lower in the combined interventiongroupvs the control group)may offer greater benefit. Telemedicine also has the potential advantage of reaching patients with suboptimal access to traditional care. Diabetic individuals with renal disease were excluded fromHINTS, likely limiting the popu...
